Take the Shot, DC Youth Vaccine Giveaway

Under the direction of Mayor Bowser, the District of Columbia (“District”) is offering DC students $51 gift cards and other gifts as tokens of appreciation, while supplies last, for taking the first shot of a COVID-19 vaccination at certain locations in the District. One (singular) parent or legal guardian of eligible students receiving their first shot will receive one $51 gift card per child vaccinated.

This week, the District is making adjustments to the Take the Shot, DC Vaccine Giveaway sites so that youth and adults can receive the vaccine and get prizes at the same sites.

Starting Saturday, October 2, the existing vaccination sites at Fort Stanton Recreation Center and Dorothy Height/Benning Library will begin offering youth vaccination incentives to students ages 12-17 who receive their first shot. Students can receive a $51 gift card or a pair of AirPods by receiving their first shot, while supplies last. Both sites are open Tuesday and Wednesday from 10 am – 6 pm, Thursday from 12 pm – 6 pm, and Saturday from 12 pm – 5 pm. The current youth vaccinations sites at Brookland Middle School, Sousa Middle School, and Johnson Middle School will cease operations on September 30.

In addition, a new vaccination site will open at Columbia Heights Education Campus (CHEC) on Tuesday, October 12, and operate Tuesday – Thursday, 4 pm – 8 pm, and Saturday, 12 pm  – 5 pm.

All DC youth can receive a free COVID-19 vaccine at these sites regardless of which school they attend. The gift cards and AirPods are available to District youth ages 12 to 17, as well as students ages 18 to 21 and currently enrolled in a DC school seeking a high school diploma.

Parents or guardians who bring their children to one of these three sites are also eligible to receive one $51 VISA gift card per child who gets vaccinated.

Student Eligibility

During the gift incentive time period, a student is eligible to receive a gift card or a gift if all the following requirements are met:

  • The student is enrolled in a school in the District of Columbia, including enrollment at a District public, public charter, private, parochial, or independent school. This includes any student enrolled at a District Local Education Agency (LEA) but who attends a non-public placement outside the District.
  • The student is a resident of the District of Columbia (which includes a student experiencing homelessness or a ward of the District who lives outside the District).
  • The student is 12-17 years of age OR is 18-21 years of age and currently enrolled in a DC school (as defined above) seeking a high school diploma.
  • A student being vaccinated between the ages of 12-17 must be accompanied by a parent/legal guardian to receive their vaccination.
  • If under 18, the student’s parent or legal guardian consents to the student’s receipt of a gift.
  • The student receives their first dose of the COVID-19 vaccine at one of the designated locations during the gift incentive time period.
  • A student as defined herein may only receive one gift card or other gift – they may not receive both. If a student chooses to accept the other gift, they may NOT also receive a gift card. If a student accepts the other gift and is a dependent student under the age of 18, their parent or guardian may still receive a gift card pursuant to the eligibility listed below.

Types of student ID accepted include the following:

  • Student School ID
  • DC One Card
  • Kids Ride Free Card
  • Report Card
  • Other School ID
  • Other Proof of Enrollment

Parent/Legal Guardian Eligibility

During the gift incentive time period, one (1) parent or legal guardian of a dependent student is eligible to receive a $51 gift card if all the following requirements are met:

  • Their dependent student is enrolled in a school in the District of Columbia, including enrollment at a District public, public charter, private, parochial, or independent school. This includes any student enrolled at a Local Education Agency (LEA) but who attends a non-public placement outside the District.
  • Their dependent student is a resident of the District of Columbia, which includes a student experiencing homelessness or a ward of the District who lives outside the District.
  • Their dependent student is under 18 years of age.
  • Their dependent student receives at least their first dose of the COVID-19 vaccine during the gift incentive time period and at one of the designated locations.
  • The parent or legal guardian presents a state issued identification card that includes a date of birth; the full legal name, address, and unique ID on that card will be recorded. The state issued identification must be currently valid or have been valid as of March 11, 2020. In lieu of a state issued identification card, parents or legal guardians may produce one or more of the acceptable documents requested by on-site officials. No gift cards will be issued to a parent or legal guardian without identification or acceptable documentation.
  • Parents or legal guardians may receive gift cards for more than one eligible dependent student.
  • Only one parent or legal guardian may receive a gift card in connection with an eligible dependent student.

Students or parents/legal guardians who do not meet the eligibility requirements defined in these Official Rules are not eligible to receive a gift or gift card. The student, parent/legal guardian, and any potential beneficiary may not assert any right or remedy on account of any disqualification, or other action. The District will determine, in its sole and absolute discretion, if a student or parent/legal guardian will be allowed to receive a gift or gift card.

Gift incentives, incentive locations, and the incentive time period are subject to change or modification as determined by the District government. The District reserves the right to reschedule any locations, times, and dates, or any aspect of the gift incentive without advance notice. Quantities of gift cards and gifts are limited, and no student or parent/legal guardian is entitled to receive a gift card or gift under this program.

Gift incentive recipients and their parents or legal guardians must sign any forms, releases of liability, and affidavits of eligibility required. Students and their parents or legal guardians understand that by accepting a gift, they reveal the medical information that the student has been vaccinated, and they reveal the information that the person is a student in the District of Columbia.

Acceptance of the gift constitutes express permission by the student and their parent or legal guardian for the District and its agencies to use the student’s name and likeness worldwide, in perpetuity, for purposes of promoting vaccination and public health, without further compensation, notification or permission unless prohibited by law, in any and all media now known or hereafter devised.

By accepting a prize, each student and their parent or legal guardian agree to hold the District, including its Mayor, any of its employees, commissioners, owners, directors, officers, affiliates, subsidiaries, contractors, volunteers, assignees and agents, absolutely harmless against any and all claims and/or liability arising out of the ownership and/or use of a gift.

Each student and their parent or legal guardian assume all liability for any injury or damage caused, or claimed to be caused, by participation in the gift incentive. The District makes no warranty, guarantee or representation of any kind concerning any gift (or any portion thereof) and disclaim any implied warranty. By receiving a gift, all gift incentive recipients agree to be bound by these Official Rules.

Students and their parents or legal guardians agree and acknowledge that they are aware of the following facts: any prize, and each portion of a prize, involves certain inherent risks, dangers and hazards that can result in injury to property and/or persons, and serious personal injury, illness, or death. Students and their parents or legal guardians freely agree to assume and accept all known and unknown risks of injury (including, but not limited to, personal injury, illness, death, and property damage) arising out of any gift.

All federal and local laws, applicable taxes, and regulations apply. The student and parent/legal guardian are responsible for all taxes, including income taxes, for any gifts received.

These Official Rules are available for review and may be amended or revised at any time at the sole discretion of the District. If any discrepancy exists between these Rules and other material describing the gift incentive or anything related thereto, these Rules shall prevail.

Official Rules

Under the direction of Mayor Bowser, through the *Take the Shot, DC Giveaway* (“Giveaway”), the District of Columbia (“District”) will conduct four or more randomized drawings for prizes for newly vaccinated persons at certain locations in the District. The persons identified in the drawings will be able to claim prizes, including new cars, groceries for a year, and Metro pass for a year.

Entering the Giveaway

Beginning Saturday, June 19, 2021, and ending July 31, 2021, or another date that the District may announce, the District is offering eligible District residents age 18 and up the opportunity to enter the Giveaway.

The District will conduct a series of four or more drawings (“Drawings”; singular “Drawing”) during and after the Giveaway. You may only win once.

You may become eligible to enter the Giveaway by obtaining a vaccination for COVID-19 at one of the following Eligible Locations during the hours that the site is open.

  • Anacostia High School, 1601 16th Street, SE
  • Ron Brown High School, 4800 Meade Street, NE
  • RISE Demonstration Center, 2730 Martin Luther King Jr. Avenue, SE

The District may alter the Eligible Locations, dates, or times. For the most up to date locations, dates, and times, go to coronavirus.dc.gov/incentives.

Residents may enter the Giveaway after receiving at their first (or only) shot of the COVID-19 vaccination at an Eligible Location between June 19, 2021 and July 31, 2021, or another date that the District may announce.

Residents who attempt to enter after receiving a second or any subsequent vaccination are ineligible.

At the same place where they may receive a $51 gift card at the Eligible Locations, residents may register for the Giveaway with the District Department of Transportation (“DDOT”) or other authorized government official. ​ Starting on June 26, 2021 Entries must be received by the last closing time of the clinic on Saturday for the following week’s Drawing, or such other cutoff as may be established for a particular Drawing.

*One entry per person.* Eligible District residents can only submit one entry into the drawings. Individuals who register themselves more than one time are subject to disqualification. Receiving a $51 gift card does not disqualify you from entering the Drawing.

Eligibility and Proof of Eligibility

You are eligible to participate in a Drawing if you meet all the following requirements:

  1. You are a permanent resident of the District of Columbia.
  2. You are at least 18 years of age.
  3. You are not an employee or Officer of DC Lottery, the DC Department of Health’s immunization program or emergency response team, or an Excepted or Executive Service member of the District government, or an officer, official, contractor or employee who assisted in the creation of this Giveaway, or in carrying out any of the Drawings. Nor may winners be a child, spouse, domestic partner, child-in-law, or parent-in-law of such an ineligible employee, contractor or officer who is living as a member of an that employee's, contractor’s or officer's household.
  4. Starting on June 19, 2021, and ending July 31, 2021, or another date that the District may announce, at one of the Eligible Locations, you received your first dose of COVID-19 vaccine if receiving the Pfizer or Moderna vaccine or your only dose if receiving the Johnson & Johnson vaccine. Residents who attempt to enter after receiving a second or any subsequent vaccination are ineligible. Official Winners will receive a prize only upon completion of their second dose (when using a vaccine that requires the person receive two doses to be fully vaccinated).

All Potential Winner information and eligibility will be reviewed prior to the declaration of an Official Winner. Potential Winners’ information, including their District residency, will be validated by District officials.

By entering the drawing, or seeking to redeem a prize, you authorize the District to access vaccination records to verify that a COVID-19 vaccination has been received.

Verification of all eligibility information must be presented to claim any prize.

  • New Vehicle (Jeep Wrangler/Ford Escape/Explore Hybrid) - Quantity: 4 (1 winner per week)
  • Groceries for a Year Gift Card - Quantity: 8 (2 winners per week)

Random Drawing to Select Winners and Notification of Winners

Four times during the Giveaway, the Potential Winners of the drawings will be randomly selected from among all eligible entries. Entrants from the three locations will be combined into one pool. Entries will carry over from one drawing to the next, and the entry pool will be larger as more persons are vaccinated at each Eligible Location.

The probability of winning any prize will be determined by the cumulative number of eligible and valid entries submitted for that Drawing.

For each Drawing, the District will notify the Potential Winner by phone or text that they may have won a prize. District officials will request the Potential Winner to complete a written Take the Shot, DC Giveaway Authorization Form (“Authorization Form”) and return it as instructed. This form will be more detailed than the entry form submitted at the time of vaccination. The Potential Winner must complete the Authorization Form in its entirety, sign it, and return it to the District within ten days of the District’s initial attempt to contact them. The information on the form will be used to confirm the Potential Winner’s eligibility, validate their entry, and obtain consent to publish their identity.

If the District makes one or more reasonable attempts to contact a Potential Winner by telephone or text and the Potential Winner fails to respond within three days, the District may move on to award that prize to an alternate choice, also selected randomly from the eligible pool.

When the District government receives the Potential Winner’s completed Authorization Form within the time specified, determines that the Potential Winner meets the eligibility requirements, and obtains further verification that the Potential Winner has received their second vaccination, if needed, the District government will certify person as an Official Winner.

The District will notify an Official Winner using information available on the Authorization Form, which may include by phone, text, email, U.S. Mail, or in-person contact. The District is not required to use any specific method to notify an Official Winner.

The promptness and degree of reasonableness of efforts to obtain an Authorization Form from a Potential Winner, or of any decision to deem a Potential Winner ineligible based upon these rules or their failure to obtain a second vaccination, if necessary, shall be at the sole discretion of the District.

Subject to verification of eligibility and compliance with these Official Rules, the Potential Winners drawn and verified will be declared the Official Winners of the Take the Shot, DC Giveaway drawings.

In the event that an entry is deemed invalid or a discrepancy is found, or a Potential Winner or Official Winner does not wish to receive a prize, refuses or fails to provide verification information, or does not promptly respond to the initial contact by District officials, that winner may be deemed ineligible and an alternate may be used. If it is determined that an alternate will be used, the winner that was deemed invalid will forfeit any prizes associated with the Giveaway. In the event of a winner’s death or inability to claim the prize, the District will make a determination about the awarding of the prize. The District’s decision will be final.

Alternates will be used in the order in which they were selected (i.e. alternate one will be used first, alternate two second and so on) as vacated spots become available.

Claiming Prizes

Potential Winners will be advised, at the time of the District’s notification, of claiming procedures and deadlines. Official Winners will receive the prize only upon completion of their second dose unless they received the Johnson & Johnson vaccine.

Official Winners must claim their prize by the claiming deadline date and following the instructions as identified in the schedule relating to that particular Drawing or prize. Alternate Official Winners must claim by the claiming deadline as identified for that particular Drawing for that prize. If an Official Winner is unable to come in person to verify or redeem the prize, such as because they are physically unable to travel, the District may, but is not required to, attempt to meet the person to obtain verification or redeem the prize in person at a mutually convenient time and place.

Official Winners will sign any forms, releases of liability, and affidavits of eligibility required. Official Winners understand that by winning and accepting a prize, they reveal the medical information that they have been vaccinated.

Acceptance of the prize constitutes express permission for the District and its agencies to use the Official Winner’s name worldwide, in perpetuity, for purposes of advertising and trade without further compensation, notification or permission unless prohibited by law, in any and all media now known or hereafter devised.

By accepting a prize, each Official Winner agrees to hold the District, including its Mayor, any of its employees, commissioners, owners, directors, officers, affiliates, subsidiaries, contractors, assignees and agents, absolutely harmless against any and all claims and/or liability arising out of the ownership and/or use of a prize.

Each Official Winner assumes all liability for any injury or damage caused, or claimed to be caused, by participation in the Giveaway or use or redemption of any prize. The District makes no warranty, guarantee or representation of any kind concerning any prize (or any portion thereof) and disclaims any implied warranty. By participating in the Giveaway, all entrants agree to be bound by these Official Rules.

All materials submitted become the property of the District and will not be returned. Some prizes may be nontransferable to any other person and will be designated as such prior to acceptance of the prize.

An appropriate IRS form will be issued by the District to each Official Winner. All federal and local laws, applicable taxes, and regulations apply. The District may but is not required to provide both the prize and any applicable taxes the Official Winner would be required to pay for the prize, such as an increase to your income taxes. The Official Winner, however, remains responsible for all taxes, including income taxes, for any prize won. All amounts or values paid to the Official Winner, or paid the Official Winner’s behalf, will be included in the taxable income reported to the IRS. The Official Winner will be taxed on those amounts. The minimum required Federal and DC taxes will be withheld from all cash prizes over $5,000; further amounts may be owed.

Publication of Drawings and Results

The Official Winners who claim their prizes will be posted on the coronavirus.dc.gov/incentives website in the following format:

  • Prize
  • Entry End Date
  • Drawing Date
  • Winner Name and Ward of Residence

General Rules

All prize details shall be determined in the sole and absolute discretion of the District. Persons who do not meet the eligibility requirements defined in these official rules are not eligible to win. You and any potential beneficiary may not assert any right or remedy on account of any disqualification, or other action. The District will determine if a winner will be allowed to receive a prize.

Drawing dates and Giveaway dates are subject to change or modification as determined by the District government. The District reserves the right to reschedule any times and dates regarding any aspect of any Drawing or the Giveaway without advance notice. The District reserves the right to void any drawing without advance notice and to conduct a new drawing if the District, in its sole discretion, determines it is necessary to maintain the integrity of the drawing or for any other operational reasons.

Interference

Any attempt by a user, participant or any other individual to undermine the legitimate operation of this Giveaway is a violation of criminal and civil laws, and should such an attempt be made, the District reserves the right to seek the prosecution of and/or damages from any such person or persons as permitted by law. Furthermore, such person may be excluded from the Giveaway.

Limitations of Liability

Official Winners agree and acknowledge that they are aware of the following facts: any prize, and each portion of a prize, involves certain inherent risks, dangers and hazards that can result in injury to property and/or persons, and serious personal injury, illness, or death. Official Winners freely agree to assume and accept all known and unknown risks of injury (including, but not limited to, personal injury, illness, death, and property damage) arising out of any prize.

Neither the District, nor the Mayor, any of the District’s employees, commissioners, directors, officers, affiliates, subsidiaries, contractors, assignees and agents are responsible for late, or incomplete or misdirected entries, or responsible for incorrect or inaccurate information, whether caused by the website, or by any of the equipment or programming associated with or utilized in this Giveaway. No one has a claim for “loss of a chance” due to any errors that caused their entry to not be included in the pool of eligible entrants.

Neither the District, the Mayor, any of the District’s employees, commissioners, directors, officers, affiliates, subsidiaries, contractors, assignees and agents are responsible for any error, omission, interruption, deletion, defect, delay in operation or transmission, communication failure, regardless of cause, with regard to any equipment, systems, networks, lines, satellites, servers, computers or providers utilized in any aspect of the operation of the Giveaway, which may include without limitation, inaccessibility or unavailability of the internet, the website or any combination thereof; theft or destruction or unauthorized access of the website; any injury or damage to the participants or to any other person's computer(s) or smartphone related to or resulting from participating in this Giveaway, or downloading materials from or use of the website.

The Take the Shot, DC Giveaway Official Rules and Drawing Procedures used by the District are available for review and may be amended or revised at any time at the sole discretion of the District. The drawings will be observed by District personnel. If any discrepancy exists between these Rules and other material describing the Giveaway or anything related thereto, these Rules shall prevail.

  1. Each person who gets their first vaccination through the vaccine gift card program can only name one “vaccine buddy” who is then entitled to a card.
  2. The maximum number of $51 cards available to any individual “vaccine buddy” is 11 cards ($561 total value)
  3. To be eligible for a gift card, the “vaccine buddy” must be 18 or older and the person(s) receiving their first vaccination must also by 18 or older. The person being vaccinated must be a DC residentTo get a “vaccine buddy” gift card, you must present a state issued identification card that includes a date of birth and the unique ID on that card will be recorded along with the address of the person and full legal name. The state issued identification must be currently valid or have been valid as of March 1, 2020. No cards will be issued to a “vaccine buddy” without proper identification.
  4. Paid Ambassadors from the nonprofit partners and DC government employees whose job includes vaccine outreach may NOT get a gift card.
